Job summary

Matrix Medical Network is hiring NPs for in-home comprehensive health assessments nationwide. You will conduct visits that can last up to an hour, review medical histories, and coordinate with PCPs to close care gaps.

Services occur at patients’ homes or skilled facilities, with flexible scheduling across weekends. Competitive compensation is based on assessments completed, with potential for FT roles and a sign-on bonus.

Qualifications

• Master’s Degree and NP licensure with prescriptive authority as needed.
• Current NP licensure in state of practice with board certification.
• State license and certifications (AANP/AACN/ANCC) as applicable.
• BLS, ACLS or CPR certification current.
• Minimum 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric health assessments including history and exam.
• Review medical history, diagnoses, and treatment plans with documentation.
• Order and administer point-of-care and lab-based tests as needed.
• Coordinate with physicians, case managers and caregivers for follow-up.
• Provide services at patients’ homes or in facility settings as needed.
