Job summary

Matrix Medical Network seeks qualified Nurse Practitioners for in-home comprehensive health assessments across the United States. During visits you review health history, medications, and social factors to identify care gaps and improve outcomes.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family or Emergency NP specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, families and other personnel as appropriate.
• Provide patient education and follow-up with PCP as necessary.
• Provide services at the member’s residence (home or SNF).
