Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments. Each visit lasts about one hour and evaluates medical history, medications, social factors, and overall health to close care gaps.

You will collaborate with physicians, case managers, and staff, providing patient education, referrals, and follow-up as needed. We offer flexible scheduling and competitive pay per assessment.

Qualifications

• Must have NP licensure in state of practice, with prescriptive authority if required.
• Master’s Degree in Nursing and NP board certification.
• BLS/ACLS or CPR certification required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatment and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members to coordinate care.
• Provide patient education and follow-up as necessary.
• Deliver services at members' residences, including homes or Skilled Nursing Facilities.
