Zusammenfassung

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments. Each visit lasts up to one hour and covers medical history, current health, medications, and social factors to help close care gaps.

You will administer point-of-care and lab testing, collaborate with physicians and care teams, and provide patient education at the member’s residence, including homes or Skilled Nursing Facilities. We offer a sign-on bonus and flexible scheduling.

Qualifikationen

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS/CPR certification.

Aufgaben

• Conduct adult/geriatric assessments including history, diagnoses, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate with PCP on education and follow-up.
• Provide services at the member’s residence, including homes or SNFs.
