Job summary

Matrix Medical Network is seeking Nurse Practitioners to conduct comprehensive in-home health assessments. You will review medical history, medications, and social factors to identify care gaps and coordinate care with physicians and multidisciplinary teams.

Position offers flexible scheduling, potential full-time opportunities, and a sign-on bonus. Requires NP licensure in state of practice and valid driver’s license. Mileage reimbursement and 401(k) benefits are provided.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Strong verbal and written communication.
• Excellent computer skills and health software familiarity.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide services at members’ residences, homes or SNFs.
