Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments for members in their homes or Skilled Nursing Facilities. You will review medical history, medications, and social factors to identify care gaps and coordinate with PCPs and care teams.

The role offers flexible scheduling, mileage reimbursement, and 401(k) options. Providers can choose when to work, including weekends, with potential FT opportunities and a sign-on bonus.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in one of the NP specialties (AANP/AACN/ANCC).
• Current BLS/ACLS/CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
