Matrix Medical Network delivers in-home comprehensive health assessments to improve outcomes and reduce costs. Providers review medical history, medication adherence, and living situations to tailor care during each visit.

We seek Nurse Practitioners with NP licensure, board certification, and strong collaboration skills. Positions offer flexible scheduling and opportunities to serve high-need populations across the country.

Qualifikationen

• Master’s degree in nursing practice or equivalent required.
• Current NP licensure in practicing state with prescriptive authority or ability to obtain.
• Board certification in one of: Adult, Gerontology, Family, Emergency, or Acute Care NP.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Aufgaben

• Conduct adult/geriatric health assessments including medical history and health education.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate patient education and follow-up with PCPs.
• Provide services at members' homes or Skilled Nursing Facilities.
