Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments for adults and seniors. Visit members at their homes or Skilled Nursing Facilities to review medical history, medications, and risk factors; coordinate follow-up and referrals as needed.

Qualifications

• Active NP licensure in state of practice (prescriptive authority).
• Board certification in Adult, Gerontology, Family, or Emergency NP specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid U.S. driver’s license and auto insurance for home visit work.

Responsibilities

• Conduct adult/geriatric in-home assessments including history, diagnoses, and health education.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members.
• Provide care at members’ homes or SNFs.
• Document findings clearly following Matrix guidelines.
