Job summary

Matrix Medical Network is hiring Nurse Practitioners to perform in-home Comprehensive Health Assessments. During visits of up to one hour, you review a member’s health, medical history, medications, and risk factors at the home or SNF, and document findings per Matrix guidelines.

We offer flexible scheduling across days and weekends, with pay based on assessments completed ($90–$115 per assessment; monthly range $1,800–$11,500 for high-volume performers).

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance requirements for the state of residence.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and caregivers.
• Coordinate with PCP on education and follow-up.
• Provide services at the member’s home or Skilled Nursing Facility.
