Job summary

Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home health assessments. You will review medical history, medications, and social determinants to guide care decisions during each visit.

Responsibilities include coordinating with physicians and other care team members, ordering tests, and documenting findings to close care gaps, with services at members’ homes or SNFs.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice, with prescriptive authority if needed.
• Board certification: AANP, AACN, or ANCC in Adult/Family/gerontology specialties.
• Current BLS, ACLS, or CPR certification.
• 1 year NP experience preferred; new grads may apply.
• Valid state driver’s license and auto insurance for residence state.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide services at the member’s home or Skilled Nursing Facility.
