Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform comprehensive in-home health assessments across the United States. Providers review medical history, medications, and social factors to capture a full picture of patient health during visits that can last up to an hour.

You will work with physicians and care teams to close care gaps and improve outcomes. Responsibilities include conducting adult/geriatric assessments, ordering tests, coordinating with PCPs, and delivering care

Qualifications

• Master’s degree or commensurate experience and NP licensure are required.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• BLS/ACLS or CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, and treatment.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
