Job summary

Matrix Medical Network seeks a qualified Nurse Practitioner to perform comprehensive in-home health assessments across the patient population. Each visit can last up to one hour and focuses on health history, medication adherence, and social determinants to close care gaps.

Providers will conduct geriatric assessments, order tests, and collaborate with physicians and care teams to educate patients and arrange follow-up.

Qualifications

• NP must have current licensure in state of practice.
• 0
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and other care team members.
