Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes and SNFs nationwide. You will review health history, medications, and social factors to close care gaps and support care planning.

The role requires NP licensure with prescriptive authority, board certification in relevant specialties, and willingness to travel up to 80–90% across the region. Competitive salary and comprehensive benefits await.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Family, or Emergency care.
• Current BLS, ACLS or CPR certification.
• Able to travel up to 80-90% of the time across the assigned region.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, including homes and Skilled Nursing Facilities.
