Job summary

Matrix Medical Network is seeking licensed Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Each visit lasts up to one hour, reviewing medical history, medications, and social factors to identify and close care gaps.

Providers collaborate with physicians, case managers, social workers, and caregivers, with flexible scheduling across Sundays to Saturdays and support for licensure expansion. In-home care minimizes barriers to access.

Qualifications

• Requires NP licensure and board certification in Adult, Family, or Gerontology specialties
• 1 year NP experience preferred; new grads encouraged to apply
• Current BLS/ACLS or CPR certification required
• Valid state driver’s license and auto insurance coverage
• Strong verbal and written communication skills and professional appearance

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis, treatment, education, referrals, and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and family members
• Coordinate with PCP for patient education and follow-up
• Provide in-home or SNF-based assessments and ensure quality standards
