Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors to identify gaps in care and coordinate with PCPs. Responsibilities include point-of-care testing, patient education, and documentation in the chart.

Role offers flexible scheduling, competitive pay, mileage reimbursement, and opportunities for licensure support. Requires NP licensure and state prescribing authority.

Qualifications

• Must hold an NP license in the state of practice with prescriptive authority if required.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care NP specialties.
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance coverage for residence state.

Responsibilities

• Conduct in-home comprehensive health assessments reviewing medical history, diagnoses, and treatment.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and family caregivers to coordinate care.
• Provide patient education and follow-up as necessary; document findings per Matrix guidelines.
• Deliver services at members' homes or Skilled Nursing Facilities as required.
