Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments for adults across the nation, including geriatric care. Providers review medical history, medications, and social factors during visits lasting up to an hour, delivering high-quality care at patients’ homes or facilities.

Applicants must hold NP licensure, be willing to travel, and have strong communication skills.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority when needed
• Board certification in Adult/Family/Gerontology/Acute Care NP specialties
• BLS/ACLS/CPR certification

Responsibilities

• Conduct adult/geriatric in-home health assessments up to one hour.
• Review medical history, medications, and social determinants of health.
• Coordinate with PCPs, case managers, and other care teams.
• Provide services at member homes or Skilled Nursing Facilities.
