Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform in-home health assessments for adults and seniors. You will review medical history, medications, and social factors to improve care and close gaps, with documentation and physician collaboration.

The role requires NP licensure, state prescriptive authority, and the ability to travel extensively, including overnight trips. Competitive pay and benefits are offered with flexible scheduling.

Qualifications

• Master’s Degree or commensurate experience and NP licensure completed.
• Current NP licensure in practicing state with prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties.
• BLS/ACLS or CPR certification.
• 1 year NP experience.
• Ability to travel 80–90% of the time with overnight stays.
• Valid driver’s license and auto insurance.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and families.
• Provide in-home health assessments to close care gaps.
• Educate patients and follow up as necessary.
