Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments. You’ll review medical history, medications, and social factors to identify care gaps and improve overall health outcomes.

Assessments occur at patients’ homes or SNFs, with supportive teams and clear documentation. You will collaborate with PCPs and care teams, gain ongoing education, and have the opportunity to travel across the state with flexible scheduling and competitive

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Advanced Practice areas.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Travel up to 80-90% of the time with potential overnight stays across the state and possibly neighboring states.
• Valid state driver’s license and automobile insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with PCP, case managers, social workers, and caregivers.
• Provide services at members’ residences including homes or SNFs.
• Coordinate patient education and follow-up with physicians.
