Job summary

Matrix Medical Network seeks qualified Nurse Practitioners to perform comprehensive in-home health assessments nationwide. Each visit lasts up to one hour and covers medical history, medication adherence, and home risks to close care gaps and improve outcomes.

Providers collaborate with physicians, social workers, and care teams, delivering care at the member’s residence, including homes and skilled nursing facilities. Applications welcome from experienced NPs with valid state licensure.

Qualifications

• Must be a licensed NP with prescriptive authority in the practice state.
• Strong communication skills and ability to educate patients and families.
• Proven clinical assessment experience with geriatric populations.
• Experience with electronic health records and basic software tools.

Responsibilities

• Conduct comprehensive health assessments in member homes or facilities.
• Review medical history, medications, and social factors to identify care gaps.
• Coordinate with PCPs, case managers, and care teams for referrals.
• Provide patient education and follow-up on treatment plans.
• Document findings clearly following Matrix guidelines.
