Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for adults and geriatric patients. You will review medical histories, medications, and caregiver input, order tests as needed, and coordinate with PCPs and case managers.

Visits occur at patients' homes or SNFs. The role requires NP licensure, board certification in relevant specialties, and travel up to 90% with flexible scheduling.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority ability.
• AANP, AACN, or ANCC board certification in Adult/Family or Gerontology specialties.
• BLS/ACLS/CPR certification.
• 1 year of NP experience.
• Ability to travel up to 80-90% including overnight travel.

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnoses, treatment and referrals.
• Administer and order point-of-care and lab testing.
• Collaborate with PCPs, case managers and other healthcare professionals.
• Provide in-home or SNF visits at members' residences.
• Document findings per Matrix guidelines and ensure clear, timely follow-up.
