Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments across the United States. You will evaluate medical history, medication adherence, and social determinants to identify care gaps and tailor health plans.

Providers administer testing as needed, collaborate with PCPs and care teams, and deliver care at homes or Skilled Nursing Facilities. Strong NP credentials, licensure, and a driver’s license are required, with flexible scheduling

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Gerontology Primary Care, Family, Emergency, or Acute Care NP specialties.
• Valid state driver’s license and automobile insurance coverage.
• Strong computer skills and experience with health records software.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and other healthcare team members.
• Provide services at the member’s residence or in Skilled Nursing Facilities.
• Coordinate patient education and follow-up with PCPs.
