Job summary

Matrix Medical Network is hiring Nurse Practitioners to perform in-home comprehensive health assessments. You will review health histories, medications, and social factors to close care gaps, coordinating with physicians and care teams. Work may occur in patients' homes or Skilled Nursing Facilities, with flexible scheduling.

The role requires NP licensure, board certification, and state prescribing authority where needed. Competitive pay, benefits, and support for licensure are provided.

Qualifications

• Master’s degree in nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in an adult-gerontology, family, or acute care track.
• Valid state driver's license and automobile insurance where required.
• Strong communication and computer skills; ability to work independently.

Responsibilities

• Conduct adult/geriatric health assessments in home or SNF settings.
• Review medical history, medications, and social determinants; document per Matrix protocols.
• Coordinate with PCPs, case managers, and care teams to close care gaps.
• Order point-of-care labs and tests as appropriate; provide patient education.
