Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments across the United States. You’ll review medical history, medications, social factors, and risk factors during visits that may take up to an hour to complete.

Responsibilities include adult/geriatric assessments, testing, referrals, and collaboration with PCPs and care teams, delivering high-quality care at patients’ homes or SNFs.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in practice state with prescriptive authority.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Automobile insurance coverage for resident state or ability to obtain.
• Strong computer skills and knowledge of medical/health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, including homes or skilled facilities.
