Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications and social factors to close care gaps. You will administer tests, coordinate with care teams, and provide education during patient visits at homes or SNFs.

Ideal candidates hold NP licensure, state authorization for prescribing, and board certification with strong communication skills.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification in adult or family specialties
• Current BLS/ACLS or CPR certification
• Travel up to 80-90% of time including overnight travel

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses and treatment
• Administer and order point-of-care and lab-based tests as needed
• Collaborate with PCPs, case managers, social workers and family members
• Provide services at members' residences (home or SNF)
• Ensure documentation aligns with Matrix guidelines and protocols
