Job summary

Matrix Medical Network in the United States is hiring Nurse Practitioners to conduct comprehensive in-home health assessments for diverse patient populations. You will review medical history, medications, and social factors during visits that can last up to an hour.

Qualifications

• Must hold NP licensure in the state of practice.
• Master's degree in nursing or equivalent experience.
• Board certification in relevant NP specialty.
• Strong communication and documentation skills.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric primary assessments and document findings.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Educate patients and follow up with care plans.
• Provide services at members' homes or facilities.
