Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments for adults, including review of medical history, medications, and functional risk factors. Visits occur at members’ residences or skilled facilities and require thorough documentation per Matrix protocols.

Each visit lasts up to one hour, with opportunities for full-time roles and licensure support, ongoing education, and scheduling flexibility to fit your availability.

Qualifications

• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
• Current NP licensure in state of practice with prescriptive authority as needed.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers; document per Matrix guidelines.
• Coordinate with PCP for patient education and follow-up; ensure appropriate follow-up.
