Job summary

Matrix Medical Network is seeking NP providers to perform in-home comprehensive health assessments across the country. You will review medical history, current health status, and medications, and coordinate with care teams to close care gaps.

Each visit includes testing as needed and documentation per Matrix guidelines. This role offers flexible scheduling and home-based patient care, with opportunities for full-time status and career advancement.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice.
• Board certification in Adult/Family NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Strong computer skills; familiarity with health record software.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments with medical history, diagnosis and treatment documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other health personnel.
• Educate patients and follow up with PCPs as necessary.
• Provide services at the member’s residence, which may be homes or Skilled Nursing Facilities.
