Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or SNFs. You’ll collect medical histories, evaluate health risks, and document findings per Matrix guidelines.

Responsibilities include education, care coordination, and cross-team collaboration with PCPs. Travel up to 80–90% within the assigned state; flexible scheduling and potential FT opportunities offered.

Qualifications

• Master's degree in nursing and NP licensure.
• Current NP licensure in state of practice.
• Board certification in an acute or primary care NP specialty.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation.
• Administer or order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers and care teams to coordinate care.
