Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by experienced NPs. You will assess health status, med history, and adherence, visiting members at their homes or SNFs.

This role emphasizes care coordination with physicians and care teams, ensuring thorough documentation and patient education. The position supports flexible scheduling and high travel, with opportunities to advance through onboarding, preceptorship, and continued education.

Qualifications

• Master’s Degree or commensurate NP experience.
• Current NP licensure in state of practice.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states.
• Valid state driver’s license for the state of residence and automobile insurance.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and other professionals as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide care at the member’s residence, which may be homes or Skilled Nursing Facilities.
