Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments across states. You will review medical history, medications, and risk factors, enabling better chronic condition management and closing care gaps during each visit.

Roles involve travel up to 80–90% with schedules based on availability, including weekends. Competitive compensation reflects the volume of assessments completed, with support for licensure and ongoing education.

Qualifications

Master’s Degree or commensurate NP experience.
Current NP licensure in state of practice (with prescriptive authority if needed).
AANP, AACN, or ANCC board certification in Adult, Gerontology, Family, or Acute care.”
Current BLS, ACLS or CPR certification.
1 year of NP experience.
Able to travel 80–90% for extended periods including overnight travel.
Valid state driver’s license and auto insurance for residence state.
Strong computer skills and familiarity with health/medical record software.

Responsibilities

Conduct adult/geriatric health assessments, medical history, diagnosis and treatment documentation per Matrix guidelines.
Administer and order point-of-care and lab testing as needed.
Collaborate with physicians, case managers, social workers and caregivers.
Coordinate with PCP on patient education and follow-up.
Provide services at members’ homes or Skilled Nursing Facilities.
