Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform in‑home health assessments. You will review medical history, medications, and risks to close care gaps, and document findings per Matrix guidelines. Travel to members’ homes or SNFs is required.

The role offers flexible scheduling, competitive pay, and opportunities for licensure support and continuing education. Training includes onboarding and a dedicated preceptor to ensure high-quality care.

Qualifications

• Master’s or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency or Acute Care.
• BLS/ACLS certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric health assessments with thorough medical history and follow‑ups.
• Administer point‑of‑care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
