Job summary

Matrix Medical Network seeks a licensed NP to perform Comprehensive Health Assessments in members' homes nationwide, including seniors and at‑risk individuals. You will review health history, medications, and social factors; provide education and referrals; and document findings according to Matrix protocols.

The role offers flexible scheduling, full-time opportunities, and support for licensure expansion, with travel up to 80–90% and potential state-to-state work arrangements.

Qualifications

• Master’s degree in nursing and NP licensure in practice state.
• Current NP licensure with prescriptive authority.
• Board certification in one of the adult-gerontology, family, or acute care specialties.
• BLS/ACLS or CPR certification.
• Minimum 1 year NP experience.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and caregivers to coordinate care.
• Provide in-home visits at members' residences, including SNF settings, with clear documentation.
