Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments nationwide. Each visit lasts up to one hour and covers medical history, medication adherence, and social factors to identify care gaps and improve outcomes.

You will coordinate with PCPs, case managers, and family members, provide education, and conduct point-of-care testing as needed. Competitive compensation, flexible scheduling, and opportunities for full-time roles are offered.

Qualifications

• Master’s degree or commensurate experience required.
• Current NP licensure in state of practice with prescriptive authority where applicable.
• AANP, AACN, or ANCC board certification in relevant NP specialties.
• BLS/ACLS/CPR certification current.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver's license and automobile insurance.
• Willingness to obtain licensure and credentialing in neighboring states as needed.

Responsibilities

• Conduct adult/geriatric assessments covering medical history, diagnosis, and treatment per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate patient education and follow-up with PCPs.
• Provide services at the member’s residence, including homes or SNFs.
