Job summary

Matrix Medical Network seeks nurse practitioners to perform in-home health assessments for adult and geriatric patients. You will review medical history, medications, and risk factors during visits at the member's residence, coordinating with PCPs and care teams to improve outcomes.

Qualifications

• Master’s degree in nursing or commensurate experience.
• NP licensure in state of practice.
• Board certification in Adult/Family NP or related.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads may apply.
• Valid state driver’s license.
• Auto insurance coverage for state of residence.
• Strong computer skills and familiarity with medical records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric health assessments.
• Review health history, medications, and social factors.
• Order and administer point-of-care and labs.
• Collaborate with physicians, case managers, and caregivers.
• Provide services at member residences (homes or SNFs).
