Job summary

Matrix Medical Network is hiring Nurse Practitioners to perform Comprehensive Health Assessments in members' homes across various locations. You will review health history, medications, adherence, and risks, and collaborate with PCPs and care teams to improve outcomes.

Providers work on a flexible schedule, with opportunities for full-time roles and state licensure support. Mileage reimbursement and benefits are included, with ongoing onboarding and education support.

Qualifications

• Master’s degree or commensurate NP experience and successful NP licensure.
• Current NP licensure in state of practice with prescriptive authority if required.
• Board certification in Adult, Gerontology, Family, Emergency, or acute care specialty.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate patient education and follow-up with PCPs.
• Provide services at the member’s residence, in homes or SNFs.
