Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or facilities. Each visit lasts up to one hour and includes review of medical history, medications, and risk factors to identify care gaps.

You will collaborate with physicians and care teams to improve outcomes. We offer flexible scheduling, competitive pay based on completed assessments, mileage reimbursement, 401(k), and ongoing education.

Qualifications

• Master’s degree in Nursing and NP licensure in state of practice.
• Current board certification in Adult/Adult Gerontology or Family practice specialties.
• Active BLS/ACLS or CPR certification required.

Responsibilities

• Perform adult/geriatric health assessments in home or facility settings.
• Collect medical history, diagnoses, treatments, and education for patients.
• Order/perform point-of-care tests and coordinate with care team.
• Collaborate with PCPs, case managers, and caregivers for follow-up.
• Document findings clearly per Matrix guidelines.

1 year NP experience (preferred)

Valid driver’s license
