Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by Nurse Practitioners, focusing on adult and geriatric populations. Assessments cover medical history, medications, and social factors, with testing and referrals as needed.

Work primarily in members' homes or facilities, ensuring high-quality, coordinated care. Providers enjoy flex scheduling, weekend options, and earnings linked to visit volume, plus licensure support and ongoing education.

Qualifications

• NP licensure in practicing state is required or obtainable
• Board certification in approved NP specialties
• Master’s degree in Nursing (or commensurate experience) and active NP license
• BLS/ACLS certification
• Valid state driver’s license and auto insurance coverage

Responsibilities

• Conduct adult/geriatric health assessments in-home or at SNFs
• Review medical history, medications, and social determinants of health
• Order/perform point-of-care and lab-based testing as needed
• Coordinate with PCP, case managers, and caregivers for follow-up
• Provide care at member’s residence and document according to Matrix guidelines
