Job summary
Matrix Medical Network is expanding its in-home health assessment program across the United States. As a Nurse Practitioner, you will perform comprehensive health assessments in patients' homes or SNFs, reviewing medical history, medication adherence and social factors.
The role offers flexible scheduling, competitive pay per assessment, and opportunities to obtain additional licensure and state credentials, with support for onboarding, education, and preceptorship.
Qualifications
- Master’s Degree or commensurate experience with NP licensure.
- Active NP licensure in state of practice with prescriptive authority.
- AANP/AACN/ANCC board certification in relevant NP specialty.
- Current BLS/ACLS or CPR certification.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license and auto insurance.
Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnoses, and referrals.
- Administer point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, social workers, and families.
- Coordinate with PCP on education and follow-up.
- Provide services at member residences (home or SNF).
