Job summary

Matrix Medical Network seeks skilled Nurse Practitioners to perform comprehensive in-home health assessments across assigned regions. Each visit may last up to one hour and focuses on medical history, medications, social determinants, and health risks.

Providers coordinate with PCPs, order tests, and deliver care in patients' homes or Skilled Nursing Facilities. We offer flexible scheduling, competitive pay per assessment, and support for licensure and continued education.

Qualifications

• Master’s degree or equivalent NP experience and licensure.
• NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family or Emergency.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family and caregivers.
• Coordinate with PCP on patient education and follow up.
• Provide services at the member’s residence (home or SNF).
