Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medications, and risks, and coordinate with physicians and care teams. Assessments are conducted at the member’s home, including testing and education.

You’ll collaborate with PCPs and other clinicians to close care gaps, provide patient education, and support ongoing management while traveling to member residences across our service area.

Qualifications

• Master’s Degree in Nursing or equivalent experience.
• Current NP licensure in practicing state with prescriptive authority.
• Board certification in Adult, Family, or Gerontology specialties preferred.
• Current BLS/ACLS or CPR certification.
• About 1 year NP experience preferred; new grads welcome.

Responsibilities

• Perform adult/geriatric health assessments in members' homes.
• Review medical history, medications, and risk factors; document findings.
• Order and interpret point-of-care and lab tests as needed.
• Coordinate with PCPs, case managers, and other care team members.
• Provide education and referrals; support care coordination.
• Work with families and caregivers to ensure follow-up and adherence.
