Job summary

Matrix Medical Network delivers in-home clinical assessments nationwide, empowering clinicians to improve health outcomes for older adults and at-risk individuals. As an NP, you will conduct comprehensive in-home assessments, review medical history and medications, administer testing, and coordinate with PCPs and care teams.

You will provide services at members’ residences, including homes and Skilled Nursing Facilities, with flexible scheduling across Sundays through Saturdays.

Qualifications

• Master’s Degree in NP or commensurate experience with successful NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain it as needed.
• Board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members during visits.
• Provide services at member residences, including homes and Skilled Nursing Facilities.
