Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments for members in their homes or Skilled Nursing Facilities. Each visit focuses on health, history, medication adherence and risk assessment, aiming to close care gaps and improve outcomes.

Providers work with physicians, case managers and staff, receiving onboarding, ongoing education and potential state licensure support.

Qualifications

• Must have NP licensure in state of practice with prescriptive authority
• Current BLS, ACLS or CPR certification
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and family members
• Provide services at the member’s residence (home or Skilled Nursing Facility)
• Support patient education and follow-up with PCP as necessary
