Job summary

Matrix Medical Network delivers in-home comprehensive health assessments through a national network of clinicians. Providers review health history, medications, and environmental factors to identify gaps in care and improve outcomes.

This role supports aging and at-risk populations with extensive collaboration among PCPs, case managers, and care teams. Providers administer tests and visit members at home or in SNFs, enjoying flexible scheduling, competitive pay based on assessments completed,

Qualifications

• NP licensure in the state of practice and prescriptive authority as needed.
• 1 year NP experience preferred; new grads encouraged to apply.
• Current BLS/ACLS or CPR certification is required.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment, education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide services at the member's residence (home or Skilled Nursing Facility).
