Job summary

Matrix Medical Network seeks qualified Nurse Practitioners to perform comprehensive in-home health assessments for members across the United States. Visits last up to one hour and cover medical history, medications, adherence, social factors, and risks to improve quality of care and close gaps in care.

Providers collaborate with PCPs and care teams, offering education and referrals. Flexible scheduling, including weekends, and strong support for continuing education are highlighted.

Qualifications

• Current NP licensure in state of practice.
• Prescriptive authority or ability to obtain.
• Board certification in NP specialty (Adult/Family).
• BLS/ACLS/CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Provide services at the member’s residence (home or Skilled Nursing Facility) and coordinate follow-up with PCP.
• Educate and follow up with patients and caregivers as part of care plans.
