Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments for adult and geriatric members. Each visit lasts up to an hour, reviewing medical history, medications, and social factors to identify care gaps and tailor plans.

You will administer and order point-of-care and lab tests, collaborate with physicians, case managers, and families, and provide care at the member's residence, including home or skilled nursing facilities.

Qualifications

• Master’s degree in NP or commensurate experience required
• Current NP licensure in state of practice with prescriptive authority
• Board certification in eligible NP specialty (AANP, AACN, or ANCC) and BLS/ACLS

Responsibilities

• Conduct adult and geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines

Skills

NP licensure in state of practice

AANP/AACN/ANCC board certification

BLS/ACLS certification

Strong communication skills

1 year NP experience preferred

Education
