Job summary

Matrix Medical Network offers in-home comprehensive health assessments conducted by qualified NPs. Providers review health history, medications, and risks during visits at the member’s residence or Skilled Nursing Facilities, with testing as needed.

The role emphasizes collaboration with PCPs and care teams to close care gaps. Opportunities are full-time with flexible scheduling and potential for broader state licensure.

Qualifications

• Must hold NP licensure in state of practice and prescriptive authority if applicable.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance as required.

Responsibilities

• Conduct adult/geriatric assessments including health history, diagnoses, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide in-home or SNF visits and patient education.
