Job summary

Matrix Medical Network offers in-home comprehensive health assessments delivered by NP professionals. The role emphasizes travel to patient homes and skilled nursing facilities to review medical history, medications, and social determinants of health.

You’ll work with physicians and care teams to close care gaps. Our program supports ongoing education, preceptorship, and opportunities to advance to full-time positions with sign-on incentives.

Qualifications

• Master’s degree or commensurate NP experience with licensure.
• Current NP licensure in the practice state with prescriptive authority.
• Board certification in Adult, Family or Gerontology specialties.
• BLS/ACLS/CPR certification current and valid.

Responsibilities

• Perform adult/geriatric assessments including medical history and diagnoses.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers and family caregivers.
• Educate patients and follow up with necessary referrals.
• Provide services at members' residences, including homes and SNFs.
