Zusammenfassung

Matrix Medical Network provides in-home health assessments and compassionate care to older adults and at-risk individuals across the nation. As an NP, you will perform comprehensive health evaluations in members’ homes, reviewing medical history, medications, and adherence to identify gaps in care.

Work is FT with flexible scheduling; opportunities for state licensure support and continued education are available.

Qualifikationen

• Master’s Degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in relevant NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and automobile insurance.
• Strong computer skills and familiarity with EMR software.

Aufgaben

• Conduct adult/geriatric health assessments during home visits.
• Review health history, medications adherence, and social environment to identify risks.
• Collaborate with physicians, case managers, social workers and care teams.
• Document findings per Matrix guidelines and coordinate follow-ups and referrals.
