Job summary

Matrix Medical Network provides in-home comprehensive health assessments across the nation, delivering high-quality care to older adults and at-risk individuals in their homes or SNFs.

As an NP, you will conduct one-hour visits, review medical histories, medications, and risks, and collaborate with a care team to close gaps in care. The role offers flexible scheduling, competitive pay per assessment, and opportunities for licensure support.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult or Family NP specialties (AANP/AACN/ANCC).
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnoses, treatments, and education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for education and follow-up.
