Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for aging and at-risk adults. Each visit reviews medical history, medications, and adherence, with documentation following Matrix protocols.

Roles include coordinating care with physicians, case managers, and family members to identify gaps and improve outcomes. You will conduct visits at members' homes or Skilled Nursing Facilities, provide education, and order tests as needed.

Qualifications

• Master’s Degree in Nursing or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family or Emergency care.
• Current BLS/ACLS CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis, and treatment planning.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, including homes or SNFs.
